In this article, the first case study of escorting catheterisation in a patient with T4 complete paraplegia and total incontinence is presented. The neurological level of the patient was T4, ASIA A. As, there has been difficulty to keep the patient on intermittent sterile catheterisation, urethroscopy was performed and any kind of narrowing or false route were not found. Afterwards, we continued intermittent sterile catheterisation, but catheterisation was still rather difficult. Meanwhile the patient found that after inserting the first catheter in the urethra, the second one waseasily reaching the bladder and emptying the bladder completely. We called the first one as `escorting catheter` because it probably occludes any functional false route to let second one pass.
