Herein, we reported a 23-year-old female patient who developed foot drop after losing 88% of her excess weight within six months after having laparoscopic sleeve gastrectomy. Electrophysiological studies showed total common peroneal nerve involvement. Within three months, the patient showed a full functional and electrophysiological recovery after receiving conservative treatment. This case emphasizes the significance of early diagnosis, as well as the potential role of corticosteroids in individualized, nonsurgical management of peroneal neuropathy in bariatric patients.
